WHAT SUPPORT CAN WE OFFER?
Crossroads Care can provide a trained carer support worker to look after the person you care for at home, to enable you to have a break. The amount of time we can offer can be varied to meet your needs and may be on either a regular weekly or planned basis, or just occasionally, for example to attend an appointment or a special social event. We provide support in lots of different ways and for lots of reasons. These can include: help with personal care/hygiene, help to prepare meals/feeding, help with accessing local community activities, assistance with medications.

HOW DOES IT WORK?
We are very happy to take referrals or enquiries about support from carers, as well as from health or social care professionals. If we are able to help, a member of the care team will visit you to carry out an assessment and discuss your need for support. They will endeavour to match the allocated carer support worker with the needs and interests of the person cared for. Once care has been agreed, we will arrange for an initial visit to introduce the carer support worker before the regular visits start. We also offer overnight support to carers. At present this support is limited.

 HOW MUCH DOES IT COST?
Crossroads Care Cheshire West & Wirral is financed by different sources and we can provide a limited amount of charitable hours as well as through Health & Social Services.You may have your service funded by Adult & Older Peoples Services, other national organisation or the PCT. You may be in receipt of Direct Payments or choose to fund your own care. However your service is funded we will assure you of a high quality professional service at all times. Donations and contributions are always welcomed and needed to supplement the care hours provided.

MEDICATION
Carer support workers can assist or oversee with medication, providing a medication consent form has been
completed. You will be asked to complete and sign a medication sheet on the onset of a service and reviewed
when medication has changes.

CONFIDENTIALITY
Crossroads will ensure that all information you give will be treated in a confidential manner. All information will
kept confidential and in accordance with the Data Protection Act.
